Registration

If you are moving into Hill Field Elementary boundaries, there are a few items you will need to bring with you to complete the registration process for your student/s:

  1. Original Birth Certificate (we'll make a copy)

  2. Immunizations record

  3. Proof of residency

  4. Parent's picture I.D.

  5. Two emergency contacts

  6. School records (if you have them)

Please note that all required documentation can be conveniently submitted online via email. Simply send the necessary files to Linda Leos.

Each of the forms below are part of the registration packet and must be filled out completely before students can be placed in a class.
